 ARISE

Breast Health Awareness Outreach and Education Project - Reaching out to underserved women, including African-American and Latina populations, and to the uninsured and underinsured. ARISE continues to reach out to women with disabilities, a group that is frequently a subset of the underserved and uninsured due to the fact that people with disabilities often live in poverty and face high unemployment rates, as well as other barriers to accessing services.

 Canton-Potsdam Hospital

Breast Cancer Patient Navigation - Patient Navigator accompanies patients to medical appointments, follows-up with patients, helps to secure resources for travel, wigs and scarves, educates patients about what to expect and what tests and procedures mean, and provides moral support throughout the entire medical journey. 

 Carthage Area Hospital
Breast Cancer Outreach and Education; Empowering Women to Take Control of Their Own Health - Program provides creative outreach and educational  activities to the at-risk population, especially those living in rural areas; seniors, college students and military families and accesses affordable services for those with financial barriers. The program hosts a support group to address the needs of survivors.

Cayuga County Department of Health & Community Health Services

Breast Cancer Screening and Education - Program extends breast cancer screening and diagnostic services to women who do not qualify for the CSP services based on age, lack of family history, or financial situation. Clinical breast exams and educational outreach are provided for women ages 20 and older living in both the underserved rural and urban areas of the county. The program also provides follow-up procedures for all at-risk women who have positive screening results.
 Chenango Health Network (CHN)
Every Woman Counts - Program conducts community-based education and outreach activities in Chenango County so that more women, particularly the uninsured and medically underserved, are screened for breast cancer and receive follow-up care. A peer-to-peer group of breast cancer survivors helps to provide access to information and support. CHN assists eligible women to enroll in the Cancer Services Program and to receive clinical breast exams, mammograms, and if needed, treatment and follow-up. CHN staff also provides support and advocacy assistance.
 Crouse Hospital

Outreach to the Underserved in Onondaga County - Program includes outreach to the community which is designed to provide increased awareness of free and reduced-cost services, particularly to the underserved and educate on the importance of breast health care. Program also seeks to identify those who qualify by collaborating with physicians who may have patients under 40 years old with abnormal findings or family history. Once at-risk women are identified, the program provides mammography screening and follow-up appointment with a medical professional if a problem is detected. 

 Highland Hospital
Clinic-based Breast Cancer Outreach - This program, a collaboration among four urban, community-based health centers and one hospital-based clinic, promotes self-breast awareness and provides clinical exams and mammography and supportive services to the poor and medically underserved women and men in Rochester and Monroe County.
 Lewis County Public Health Agency
Circle of Care-Breast Cancer Navigation Program - Program will provide breast health screening and education to those women who do not qualify for these services through the Lewis County's CSP, as well as publicize the possibilities for free or low-cost screening. Women who qualify and have positive findings will receive appropriate follow-up treatment and services.The Navigation Program will provide consistent and progressive community-based navigation services, resultiing in a seamless transition to hospital navigator, if and when necessary.
 Cancer Services Program of Monroe County
Breast Health Outreach - Program provides clinical breast exams and ensures access to annual  mammograms for women who experience barriers to care such as a lack of health insurance, unawareness of available services, or an inability to seek care; teaches women how to perfrom monthly self-awareness checks; and increase awareness and education about the importance of breast cancer screening.
 Cancer Services Program of Oneida, Herkimer, and Madison Counties
CSP of Oneida, Herkimer, and Madison Counties - Program provides monthly mammogram opportunities to the women of the Oneida Indian Nation, education and awareness outreach to Latina women in Utica, screening and diagnostic services to underserved women who are not otherwise eligible for CSP services, and education, awareness, and screening services in rural areas. The program also provides supportive services and side-effect management to this at-risk population.
 Onondaga County Department of Health
Outreach and Screening Services for At-Risk Populations - Program seeks to increase breast cancer screening rates for urban African-American women, Onondaga and Oneida Nation women, and uninsured (including newly uninsured) women. Program activities follow a woman through breast cancer education, screening, diagnosis and treatment assistance. Key activities include culturally appropriate and tailored outreach and education to at-risk populations. Program reduces barriers to care by providing individualized case management and patient navigation. Program provides funds to offset financial barriers for screening and diagnostic treatment.
 St.Joseph's Hospital Lymphedema Program
L.E.A.P. from Stage 0 -Lymphedema affects thousands of women who have undergone treatment for breast cancer. This program educates medical providers treating lymphedema patients and offers tools to make treatment accessible to their patients. L.E.A.P. also provides treatment and diagnosis through pre-operative baseline measures and education, bandages and garments to the uninsured and under-insured, self-management techniques, rehabilitative exercise, and educational materials/classes.
 St. Lawrence County Public Health Department
St. Lawrence County Cancer Services Program - Provides comprehensive, age-appropriate breast cancer screening to at-risk population in St. Lawrence County, remove financial barriers for care to those women who are identified as needing follow-up testing for breast cancer, and covers for treatment needs and services to those qualifying women who are diagnosed with breast cancer.
 Spanish Action League
Latinas al Mando Breast Health and Cancer Prevention Campaign- Program designed to increase access to breast health information and care by conducting information workshops, providing medical interpreters for medical appointments, and airing information on radio programs in Spanish that target the Latina community.
